Hours: Mon - Fri: 8am to 4:30pm
EMERGENCIES
Physicians are available for emergencies that may arise after or before regular office hours. Please call 785-841-2280 and our answering service will receive your call and contact the physician on-call.
CANCELLATIONS
It is requested that you call twenty-four hours in advance if you are unable to keep your scheduled appointment.
INSURANCE
Lawrence Eye Care Associates, P.A. participates with Medicare and a variety of insurance plans.
When making an appointment, please mention your insurance carrier. We can inform you of our level of participation with your carrier, and whether a special referral is needed from your primary care doctor.
PAYMENT
Payment is requested at the time of service. If the doctors are participants with your insurance company, we will file the claim for you and bill you for any balance due after insurance payment.
Any co-payments due will be collected at the time of the service. If the doctors do not participate with your insurance company, we will provide you with a patient care charge slip to submit to your insurance company.
If you have any questions regarding payment for services or your bill, whether it involves private pay or insurance reimbursement, please call 785-841-2280.
ALL PATIENTS
Please bring all insurance information with you to each appointment, as well as a listing of current medications you are taking, both prescription and non-prescription.
If you are a new patient we will ask you to complete our information packet prior to your appointment. All patients will be asked to update any changes in their records since their last appointment.
MEDICAL RECORDS
Medical records are strictly confidential. Lawrence Eye Care Associates, P.A., requires that you sign an authorization form, before any requests for copies of your records can be fulfilled.
OUR PHYSICIANS
Dr's. Lynn W. O'Neal, Mary P. Lange, Natalie M. Koederitz, and M. Scott Hickman are members of the American Academy of Ophthalmology. Our physicians are certified or board eligible by the American Board of Ophthalmology.
